Regional teleradiology — from image to second opinion
A Central Medical Image Archive (CMIA), study routing between healthcare facilities and centers of expertise, asynchronous and online consultations, case conferences. A secure perimeter, integration with EGISZ (Unified State Health Information System of Russia) and regional radiology information systems (RRIS).
A regional archive for thousands of beds and hundreds of facilities
The CMIA collects studies from every facility in the region — hospitals, outpatient clinics, rural health posts (FAPs), mobile X-ray units. A single source of truth for the regional diagnostic radiology service and a single point of integration with EGISZ and VIMIS (vertically integrated medical information systems).
Centralized storage. DICOM objects from all facilities in the region, a single patient identity, series deduplication.
Gateway for unstable links. Queuing, resumption of interrupted transfers, compression — works over links from 2 Mbit/s, including from FAPs.
De-identification and encryption. DICOM tag masking, AES encryption at the storage level, a secure perimeter via ViPNet by Infotecs.
Long-term preservation. Hierarchical storage, replication to a backup site, checksum-based integrity control.
Export to EGISZ. Automatic publication of electronic medical documents, IHE XDS-I, data transfer to the VIMIS systems “Prevention”, “Oncology” and “AKiNEO” (obstetrics, gynecology and neonatology).

Routing and quality control by competency
OpenRIS automatically matches a radiologist to each study: modality, body region, SLA, workload, qualifications. A random sample goes to the quality control department. Service and cost accounting is built in.
Connecting remote radiologists. Authorization, competency profile, shift schedule, mobile worklist.
Competency-based auto-routing. Rules: modality, condition (nosology), SLA, CITO priority, the physician’s qualifications and current workload.
Internal quality control. Random sampling algorithm, double reading, a discrepancy log and a QC index for each physician.
Service and cost accounting. Rates by modality and condition, billing for the requesting facility, reports for the territorial compulsory health insurance fund (TFOMS) and the Ministry of Health.
BI analytics. Reading time, SLA deadlines, workload by shift, discrepancy rate, dashboards for the chief physician and the regional chief radiology specialist.
Connecting third-party healthcare organizations. An open API for commercial clients and partner centers — no software reinstallation.
Module 03 · Telemedicine
Consultations, case conferences and second opinion
All telemedicine scenarios under Federal Law No. 323-FZ “On the Fundamentals of Health Protection of Citizens in the Russian Federation” in a single tool. Asynchronous consultations read within the SLA, online consultations with a shared viewer window, case conferences with several experts, monitoring of physiological parameters.
Asynchronous consultation. The requester sends a study, the expert reads it by the set deadline and signs the report with a qualified electronic signature (QES).
Online consultation. A video call with a shared DICOM viewer — the physician and the expert move the cursor and scroll through slices in sync and discuss by voice.
Physician case conference. Several experts in one window, voting on the conclusion, a final report signed by all participants.
“Doctor — requester” chat. Linked to the study, history from the moment the request is created, attachment of DICOM series and signed reports.
Remote monitoring. Collection of physiological parameters from connected devices, threshold alarms, transfer to the HIS (hospital information system).
